Clinical Scenario


Imagine an older client recovering at home after a hospital stay. Without streamlined ordering, receiving these items could be delayed, affecting recovery. Quick Guide: How to Apply for SWEP


Applying for SWEP doesn’t have to be complicated. Here’s the process in six easy steps:


Referral – A registered health professional (e.g. OT, physio) initiates the process.

Assessment – The clinician conducts a detailed needs assessment at the client’s home to confirm the best solution.

*The SWEP Picklist has been designed to fast-track reporting as these products have already been clinically approved by Therapists.

Prescription & Submission – Select equipment from the SWEP Picklist, match the correct SWEP catalogue code, and submit with therapist report of justification.

Prioritisation – SWEP reviews applications against urgency, guidelines, and funding allocation.

Notification & Approval – Clinician and client are informed of the outcome.

Delivery & Support – Approved items are supplied by Crescent Healthcare.
